Community Habilitation

 

 

 

The goal of this program is to empower people with developmental disabilities to obtain the support needed to achieve greater independence, at home and in the community. The program provides one-on-one service to all participants in order to obtain new skills or polish already acquired skills, by assisting participants to identify personal goals and develop a tailored action plan to help navigate their journey to independence and a fulfilling life.

 

In contrast to traditional facility-based programs, we work with individuals in their own communities, teaching them how to access community resources, engage and socialize in local activities and events, and connect individuals with local volunteer or employment opportunities. To facilitate community inclusion, integration, and relationship, a customized "at-home" training program encourages individuals with Autism Spectrum Disorders to live to their fullest potential. This individualized training concentrates on areas of adaptive skills and behavioral support.

 

 

Supports may include:

 

  • Teach participants to access community resources.

  • Provide opportunities to socialize and enjoy activities within their community.

  • Enhance self-care skills, personal hygiene, health care skills, etc.

  • Promote positive strategies and adaptive behaviors.

  • Support plans to address challenging behaviors.
